In addition to the application packet, the following items are required prior to acceptance
to PJA:
Two Student Information forms - signed by you - to be given to and completed by two different teachers who have had contact with your child in a formal group or Academic setting
Request for School Records to be delivered by you to your child's current school
A Medical Information form, to be filled out and signed by you and returned to the PJA office
A Health Examination Information form to be delivered by you to your child's medical practitioner who will then fill it out, sign it and mail it to the PJA office
Immunization Certificate
An Evaluation Consent Form to be signed by you, which gives your consent to an assessment of your child by our staff
A current photograph of your child
After we have received all of the above information, you will be contacted to set up a time for
your child's assessment. You and your child will meet individually with faculty members for a
one-on-one appointment. Children entering third grade and above will visit PJA for a half day,
at which time faculty members will do an informal observation of your child.
PJA holds an annual Kindergarten Roundup in the spring for the upcoming school year. This is an
opportunity for incoming kindergarten students to experience a warm initial contact with peers
and teachers.
